H.A.N.A. of Tampa Membership Agreement

This Membership Contract, when signed by the applicant and accepted by Haitian American Nurses Association of Tampa, Inc (HANA of Tampa, Inc.) constitutes a binding contract between HANA of Tampa, Inc. and the individual executing the signature page of this Contract. Membership rights, privileges and obligations commence when the Contract is signed by both parties as of the “Contract Date” below.

Terms of Membership

Type: Individual Membership

• Regular Member - (ARNP/RN/LPN/LVN) all registered and licensed professionals who hold an unencumbered license with the licensing agency of their state or territory. Regular members are entitled to all privileges and benefits of membership upon receipt of membership dues outlined below. Regular Members must remain engaged in pursuits which further the purposes of the Association.
• Student Member-all actively enrolled undergraduate students in an accredited nursing program who can provide proof of enrollment. Student members are entitled to limited privileges and benefits upon receipt of membership dues outlined below. Student members may not hold office, but may serve on committees/task forces. Student Members who are licensed nurses may vote.
• Retired Nurse/Retired Nurse Volunteer-(retired ARNP/RN/LPN/LVN) all ‘Retired’ previously licensed professionals over the age of 65 years old who can provide proof of ‘retired status’. Retired members are entitled to all privileges and benefits of membership upon receipt of membership dues outlined below. Retired Members must remain engaged in pursuits which further the purposes of the Association.
• Affiliate Member- all other non-health professionals and allied health professionals who are not licensed nurses. Affiliate members are entitled to all benefits of membership upon receipt of membership dues, but may not hold office. Affiliate Members must remain engaged in pursuits which further the purposes of the Association.

Membership Dues:

• Initial one-time fee of $150 for ‘Regular Member/Affiliate Member’. Followed by subsequent yearly renewal fees of $125.
• Initial one-time fee of $100 for ‘Retired Member’. Followed by subsequent yearly renewal fees of $75.
• Initial one-time fee of $75 for ‘Student Member’. Followed by subsequent yearly renewal fees of $50.

Dues cover a 12-months period for HANA of Tampa, Inc (from January thru January). Dues are payable January 1 and are considered delinquent after April 1. Notice will be given to members upon failure to pay annual dues, and if such dues are not paid within 90 days, privilege of active membership shall be suspended. An additional $25 late fee will be assessed towards renewals for all delinquent dues paid within the allotted 90 days period.

Member Privileges, Benefits, & Obligations

• Privileges: Regular and Retired Members who remain active in the organization (‘active’ as outlined below) may hold office positions on the Executive Board, may cast votes, may participate/serve on and steer committees and task forces, may represent the organizations professionally with approval from the chapter’s Executive Board.
• Benefits: Regular, Retired, and Affiliate Members who remain active in the organization (‘active’ as outlined below) will obtain Continuing Education Units (CEU’s) when appropriate for presentations provided by HANA of Tampa, Inc, may obtain proof of membership letters when necessary, may list HANA of Tampa, Inc. on professional resume, may obtain community service hours upon providing proper/necessary documents, may obtain Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS) certification/recertification (terms of cost TBD), and may gain recognition/awards by the organization as deemed necessary.
• Obligations: All paid HANA of Tampa, Inc ‘Members’ must comply with the following to remain in ‘Active’ status in the organization. Inability to meet these obligations effectively will lead to forfeit of ‘Active Member status’ and loss of ALL benefits and privileges.
• Attend no less than 70% (no less than 8 meetings in 12-months period) of all scheduled monthly meetings.
• Participate in no less than 50% of all scheduled community service events.
• Pay membership dues within 30 days of submitting membership application.
• Pay annual membership dues in a timely manner as outlined above
• Not engage in any activity deleterious to the interests of HANA and/or its subsidiaries or members.

By signing this contract, you affirm to abide by the ‘Terms of Membership and Member Privileges, Benefits, & Obligations’ listed above. This is a binding and legal agreement between you and Haitian American Nurses Association of Tampa, Inc. Failure to fulfill any parts of this contract will result in loss of ‘Active Membership’ status and ALL privileges and benefits described above.
